2023-2024 Online Registration Directions for Returning Students
Registration for all RETURNING students (new and 4K students, please see highlighted area below) will be open August 1 and closing August 19, 2023, but if you register between August 1 and August 15, your credit card processing fees will be waived. If you choose to pay after August 15, you will be charged approx. 4% processing fees.
1) All registration forms and information are located on this page. However, we do require physical copies of the forms listed below, if applicable:
a. Physical Form or Alternate Year Card for Athletes in Grades 7-12. If you have a 6th grader that has been accepted into a particular sport by a coach, you will need to speak with the coach regarding forms and payment.
b. Prescription Medication Consent form
c. Consent for Allergy Medications
d. Consent for Inhaled Asthma Medications
e. Non-prescription Medication Consent – if your child regularly takes a nonprescription medication, we ask that you provide a bottle and sign the form.
2) To begin your online registration process, go back to our website home screen. On the right hand side, click on the Skyward tab.
3) This will take you to your Family Access login. If you do not remember your login, just click on the “Forgot your Login/Password?” to retrieve your information. If you don't know your login, please email Denise at swansde@monticello.k12.wi.us or call 938.2700.
4) Click on the “Returning Student Registration” tab to the left of the page and choose a child to register. You will need to complete this process for each child.
5) Go through each step and edit and click complete step and move to next after you have completed the step. You will then need to click on the next step in the list.
a. It is very important that you make sure all information is updated in Steps 1 (this is student information only - please do not enter your personal information here) & 3 so our records are accurate.
b. Steps will vary depending on the grade level of your student.
6) Once you have completed a step and checked the box, you will see a green check mark with the time and date stamp stating when it was completed.
7) Proceed through all of the steps (some are optional). When you get to the last step, “Complete 2023-2024 Online Registration” a summary will appear in the center of the screen.
a. It will show any steps that were not completed or skipped.
b. Once all registration steps have been completed click “Submit 2023-2024 Online Registration”.
8) If all steps have been completed correctly, you will receive a message stating Online Registration for this child is done.
9) Repeat this process for any additional children.
10) If there are any steps that have not been completed correctly, the school will contact you. It is important that each student is registered completely.
11) If you are unable to register online, please contact the school to make an appointment to register your child in person at the school. Any families new to the district or with new students will need to make an appointment to register.
12) All students must register by August 25, 2023 either online or in person prior to starting school in the fall.
