Hi!  My name is Carolyn Schwartzlow, and I am the Monticello School Nurse!  I am a Monticello Schools graduate, and I received my Bachelors of Science in Nursing from UW-Madison.  This will be my second year nursing at Monticello.    I am at school 3 days/week- Monday, Wednesday, and Friday.  My time at school is busy helping students grades 4K-12, overseeing student health care needs, and promoting general health and wellness throughout the school.

In addition to working at Monticello Schools, I also work part-time at the Madison VA in the Cardiac ICU. 

On my website, you will find links to needed Monticello Schools health forms, including a health information form and medication consents for both prescription and nonprescription medications.  There are also several links with infomation regarding immunizations, concussions, FAQs, as well as a variety of websites with great health tips for parents and kids.
